They took great care of my mom. She was there post vehicle accident and got her walking again and she ended up staying there custodial. Until she went to a board and care. But they made her feel at home and always had activities for her to do as well!! Thank you SJPA staff!!
My sister in law was rarely changed and they let her get bad diaper rash. Also,No PT at all!

To individuals being given this facility as an option for their loved one: Do your research (ProPublica and Medicare have detailed reviews and list all their reported deficiencies), advocate for your person. To the staff & administrators of this facility: would you want your own loved one cared for here? My mother was admitted here after a stroke. Over several days, she repeatedly complained of lower abdominal pain and developed diarrhea. Staff response was limited to Tylenol, and her symptoms were repeatedly minimized — at one point suggested to be “skin irritation,” despite her clearly describing internal pain. No doctor evaluated her during this time. Only after I pushed for answers did staff contact a doctor, which escalated to EMT transport to the hospital, where she was diagnosed with C. diff, a serious and highly contagious infection. Physical therapy was severely lacking. We were told therapy was limited because there were not “two men” available to assist her. As a result, she was largely bedbound, with the exception of one session — during which I had to assist the physical therapist myself while visiting. Additional concerns included extremely poor food quality, lack of dignity during care (curtains not consistently drawn), and infection control issues, including a resident meant to be in isolation wandering the hallways. My mother worsened under this facility’s care and required rehospitalization. I have filed a complaint with the California Department of Public Health. I strongly urge families to look elsewhere.
I am/was a resident of this facility I was in San Jacinto Healthcare for a year and a half. I got to love that place because I love the people and it was really hard when I left. I give five stars for everything. It was post acute. It was like three to a room. It was more like a hospital in a sense. I had a great experience there because the staff made it so wonderful. They were so caring for me as an individual, even though they had probably 100 residents. They had activities and I played bingo all the time. They had a lot of decorating for the holidays and made it really festive and fun. They also were very caring, like if I pushed my button in need, they would be right there for me. It was like home to me. The staff made everything really nice. As far as the therapy that was offered there, there was a gym, like a room that I could ride the bike. That was open at any time and I liked that, being able to get my therapy, because I couldn’t even walk when I first got there. They helped me learn to walk again. It was amazing. The room was more of an institutional setup. It was three hospital beds with curtains that pulled around. The food was great, but sometimes when it was brought to us, it would be cold.
The staff is kind. They seem to genuinely care about their patients and are interested in their wellbeing.

Very friendly and helpful staff. I came to visit a family friend, receptionist checked me in & set me up a nice area in the lobby for me and my family while we waited on our loved one to be done with therapy. Great place!

While visiting my aunt last week everyone was very helpful and pointed me to the right direction. Overall a very nice place with nice workers and a clean environment.

This facility is a rebranded San Jacinto healthcare a very terrible facility with very low quality care and maintenance Do Not be fooled

I am a friend or relative of a current/past resident Disgusting rooms unprofessional staff has no respect for personal property or dignity
I visited this facility My mom was in there for approximately 2 weeks at 83 and was on the mend of surgery. Thy let my mom dehydrate to the point of emergency help and ending up passing. I would not bring my dog there . Please save your family and don’t go to this kennel. There are very short help and the people that work there should be ashamed of your selves!
I am a friend or relative of a resident Mom is in San Jacinto Healthcare, a nursing and a rehab facility. They got my mother on the air mattress because of the wound, and she has to be turned around from time to time. The food is brought to her, and she is eating everything, so she is doing fine there. They have a place that you can either visit in a courtyard or in an open den area. They also have exercise equipment where they have to do exercises from time to time, so they can stay mobile. They are very attentive to her needs, and they answered every question that we had. If my mom needs something, they are there right away. They are polite, courteous, and professional.
